COVID-safe procedure

This form is part of Camp Simcha’s COVID-safe procedure for Staff, Contractors and Volunteers.

Due to families having medically vulnerable children, it is important for us to do our upmost to protect the families from COVID.

A few notes:

  • Everyone is required to have a Lateral Flow Test in the 24 hours preceding the meeting of a family and fill in the below form at least 1 hour prior to the visit to maintain everyone’s safety.
  • Meeting outdoors vastly reduces the risk of airborne transmission, but this may not always be possible. If you’re indoors, you should let fresh air in to reduce the risk of catching or spreading COVID. In indoor places like shops, transport or taxis, masks must be worn at all times.
  • You should have had at least 2 vaccines and if possible a booster
  • When meeting please use appropriate ways to reduce infection including use of antibacterial hand gel, not having physical contact and not sharing food.

    • I am not self-isolating in accordance with NHS Test and Trace OR UK Government guidelines (including quarantine after travelling in accordance with the latest guidance)*

    • I am not currently awaiting the result of a COVID test*

    • I have not had a positive COVID-19 test result within the last 10 days and no one in my household has*

    • I have taken a Lateral Flow Test within 24 hours of my visit and the result is negative*
